Helping Families Find a Way Forward
A medical social worker can assess barriers affecting the patient’s care and connect the family with appropriate support. Services are provided as part of the physician-ordered home health plan when medically necessary.
Support May Include
- Counseling related to illness, disability, grief or changing abilities
- Long-range care planning and decision support
- Referrals to community programs and support services
- Assistance identifying available financial or transportation resources
- Help addressing caregiver stress and family concerns
- Coordination with the home health clinical team
