From Referral to Recovery

Starting home health care may feel unfamiliar. We make the process as simple, responsive and understandable as possible.

Step 1: Start the Conversation

A patient, family member, physician, hospital, rehabilitation facility or other authorized referral source may contact Loving Light. We listen to the current concerns, review the requested services and explain what information is needed.

Step 2: Confirm Orders and Eligibility

Home health care generally requires an order from an authorized medical provider. Our team coordinates the necessary referral information, reviews clinical eligibility and verifies available insurance or payment coverage. Final authorization and coverage depend on the patient’s payer and plan.

Step 3: Complete the In-Home Assessment

A qualified clinician visits the patient at home to assess health needs, medications, abilities, safety risks, goals and the support available from family or caregivers.

Step 4: Build the Plan of Care

Working with the physician, we develop an individualized plan that identifies the services, visit frequency, treatments, education and goals appropriate for the patient.

Step 5: Deliver and Coordinate Care

The assigned nurses, therapists, social workers and aides provide care according to the plan. The team documents progress, communicates across disciplines and reports significant changes to the physician.

Step 6: Prepare for Independence or the Next Level of Care

From the beginning, we teach patients and caregivers how to manage safely between visits. As goals are met, we prepare the patient for discharge or help coordinate appropriate next steps when additional support is needed.
